Lisinopril: The Most Affordable Option

Lisinopril is consistently the least expensive ACE inhibitor and one of the cheapest medications of any kind. At $4–$10/month, it is available at Walmart's $4 program and through GoodRx/RxGo coupons at most major pharmacies. It is dosed once daily and has decades of safety and efficacy data.

ACE Inhibitors vs. ARBs: Which Is Cheaper?

ACE inhibitors (particularly [lisinopril](/drugs/lisinopril) and enalapril) are generally cheaper than ARBs. Losartan is the least expensive ARB at $10–$20/month. ARBs are preferred when patients develop ACE inhibitor cough (10–15% of patients) or angioedema.

Sacubitril/Valsartan (Entresto): The Exception

Sacubitril/valsartan (Entresto) is an ARNI — a combination of an ARB (valsartan) and a neprilysin inhibitor (sacubitril). It is preferred over ACE inhibitors for heart failure with reduced ejection fraction but costs $500–$600/month without insurance. A generic version is expected in 2025–2026. Novartis offers a patient assistance program (Entresto Together) for eligible patients.
