Alcohol and Blood Pressure Medications: What You Need to Know

Alcohol raises blood pressure and can interact dangerously with several antihypertensive medications. Here is what happens when you mix alcohol with lisinopril, amlodipine, metoprolol, and other blood pressure drugs.

How Alcohol Affects Blood Pressure

Alcohol has a complex, dose-dependent effect on blood pressure. Immediately after drinking, alcohol causes a brief drop in blood pressure (vasodilation). However, regular or heavy alcohol consumption raises blood pressure significantly — and is one of the most common causes of secondary [hypertension](/conditions/hypertension). Reducing alcohol intake is one of the most effective lifestyle interventions for lowering blood pressure.

The relationship is dose-dependent: consuming more than 2 drinks per day consistently raises systolic blood pressure by 5–10 mmHg. Heavy drinking (more than 4 drinks per day) can raise blood pressure by 15–20 mmHg — enough to negate the benefit of most antihypertensive medications.

The Biggest Risk: Orthostatic Hypotension

The most common serious interaction between alcohol and antihypertensive medications is orthostatic hypotension — a sudden drop in blood pressure when standing up. This can cause dizziness, lightheadedness, and fainting, increasing the risk of falls and injuries. The risk is highest with:

  • Alpha-blockers (doxazosin, prazosin)
  • Diuretics (especially in hot weather or with dehydration)
  • Calcium channel blockers (particularly diltiazem and verapamil)
  • Any combination of antihypertensives

Safe Drinking Guidelines for Antihypertensive Users

  • Limit alcohol to no more than 1 drink per day for women, 2 drinks per day for men
  • Never drink on an empty stomach
  • Rise slowly from sitting or lying positions after drinking
  • Stay well hydrated — alternate alcoholic drinks with water
  • Avoid alcohol in hot weather, after exercise, or when dehydrated
  • Check your blood pressure more frequently when drinking
  • Avoid alcohol entirely if taking alpha-blockers or if you have a history of fainting
