The Short Answer
Moderate alcohol consumption (1–2 drinks per day) is generally considered safe for most people taking [metformin](/drugs/metformin). However, heavy or binge drinking significantly increases the risk of lactic acidosis — a rare but potentially life-threatening complication. Alcohol also affects blood sugar in ways that can be unpredictable for people with diabetes.
The Main Concern: Lactic Acidosis
Lactic acidosis is the most serious risk associated with combining metformin and alcohol. Both metformin and alcohol can independently increase lactate levels in the blood. When combined — particularly with heavy drinking — the risk of lactic acidosis increases substantially.
Lactic acidosis occurs when lactate accumulates faster than the body can clear it, causing the blood to become dangerously acidic. Symptoms include:
- Nausea and vomiting
- Abdominal pain
- Muscle weakness or cramping
- Rapid or difficult breathing
- Dizziness and confusion
- Feeling cold or blue (cyanosis)
Lactic acidosis associated with metformin is rare overall (approximately 3–10 cases per 100,000 patient-years), but the risk increases significantly with heavy alcohol use, dehydration, kidney impairment, and liver disease. If you experience these symptoms, seek emergency medical care immediately.
Alcohol and Blood Sugar: Unpredictable Effects
Alcohol has complex effects on blood glucose that are important for people with [type 2 diabetes](/conditions/type-2-diabetes) to understand:
- Short-term (during and immediately after drinking) — Alcohol inhibits gluconeogenesis (the liver's production of glucose), which can cause hypoglycemia (low blood sugar), especially if you have not eaten
- Short-term (sugary drinks) — Beer, wine, and mixed drinks with juice or soda can raise blood sugar
- Long-term heavy drinking — Chronic heavy alcohol use causes insulin resistance and worsens glycemic control
Safe Drinking Guidelines for Metformin Users
- Limit alcohol to no more than 1 drink per day for women, 2 drinks per day for men
- Never drink on an empty stomach — always eat when drinking
- Avoid binge drinking (4+ drinks in 2 hours for women; 5+ for men)
- Stay well hydrated
- Monitor blood glucose more frequently when drinking
- Carry glucose tablets or snacks in case of hypoglycemia
- Avoid alcohol if you have kidney disease, liver disease, or [heart failure](/conditions/heart-failure)
When to Avoid Alcohol Entirely
Avoid alcohol completely if you:
- Have chronic kidney disease (eGFR <60 mL/min/1.73m²)
- Have liver disease or cirrhosis
- Have heart failure
- Have a history of lactic acidosis
- Are taking other medications that interact with alcohol
