Gabapentin (Neurontin): Answers to the Most Frequently Asked Questions

What Is Gabapentin Used For?

Gabapentin (brand name Neurontin) is an anticonvulsant medication with two FDA-approved indications:

  1. Epilepsy: As adjunctive therapy for partial seizures in adults and children over 3 years old.
  2. Postherpetic neuralgia: Pain following shingles (herpes zoster) in adults.

However, [gabapentin](/drugs/gabapentin) is widely used off-label for many other conditions, including:

  • Diabetic peripheral neuropathy
  • Fibromyalgia
  • Anxiety disorders (GAD, social [anxiety](/conditions/anxiety))
  • Restless legs syndrome
  • Alcohol withdrawal
  • Hot flashes (in menopausal women and men on androgen deprivation therapy)
  • Chronic low back pain

Off-label prescribing accounts for approximately 83% of all gabapentin prescriptions in the US.

How Does Gabapentin Work?

Despite its name, gabapentin does not directly affect GABA (gamma-aminobutyric acid) receptors. Instead, it binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This reduces the release of excitatory neurotransmitters (glutamate, norepinephrine, substance P) from nerve terminals, dampening pain signal transmission and neuronal excitability.

How Long Does Gabapentin Take to Work?

For neuropathic pain, gabapentin typically begins to provide relief within 1–2 weeks, with maximum benefit at 4–6 weeks. For epilepsy, it may take several weeks to find the optimal dose. For anxiety, some patients notice benefit within days (due to its sedating properties), but sustained anxiolytic effects develop over 2–4 weeks.

Does Gabapentin Cause Weight Gain?

Yes — weight gain is a recognized side effect of gabapentin, particularly with long-term use. Studies report weight gain of 2–5 kg in some patients. The mechanism is not fully understood but may involve increased appetite and fluid retention (peripheral edema). Weight gain is more common at higher doses and with longer duration of treatment.

Does Gabapentin Cause Memory Problems?

Cognitive side effects including memory problems, difficulty concentrating, and "brain fog" are common with gabapentin, particularly at higher doses. These effects are dose-dependent and usually improve with dose reduction. Older adults are more susceptible to cognitive side effects from gabapentin.

Can You Drink Alcohol While Taking Gabapentin?

Combining gabapentin with alcohol is not recommended. Both are central nervous system depressants, and their combination produces additive sedation, impaired coordination, and cognitive impairment. More importantly, the combination increases the risk of respiratory [depression](/conditions/depression) — particularly dangerous in patients who also take opioids. The FDA added a warning in 2019 about serious breathing difficulties with gabapentin in patients taking CNS depressants.

Is Gabapentin Addictive?

Gabapentin has abuse potential and is classified as a Schedule V controlled substance in the US (and more tightly scheduled in some states). Physical dependence can develop with regular use, and withdrawal symptoms occur if it is stopped abruptly. Gabapentin is particularly misused in combination with opioids, where it enhances the opioid "high" and increases overdose risk. Patients with a history of substance use disorder are at higher risk for gabapentin misuse.

What Are Gabapentin Withdrawal Symptoms?

Stopping gabapentin abruptly after regular use can cause withdrawal symptoms including:

  • Anxiety and agitation
  • Insomnia
  • Sweating and tremor
  • Nausea and vomiting
  • In severe cases: seizures (particularly in patients with epilepsy)

Gabapentin should always be tapered gradually rather than stopped abruptly. Your doctor can create a tapering schedule based on your current dose and duration of use.

How Much Does Gabapentin Cost Without Insurance?
