What Is Metformin?

Metformin (brand names: Glucophage, Glumetza, Fortamet) is an oral medication used primarily to treat type 2 diabetes. It belongs to a drug class called biguanides and has been in clinical use since the 1950s. Today, it is the most widely prescribed diabetes medication in the world and the recommended first-line treatment in virtually every major clinical guideline.

How Metformin Works

Unlike many diabetes medications, Metformin does not stimulate the pancreas to produce more insulin. Instead, it works through three complementary mechanisms:

1. Reducing Liver Glucose Production

The liver continuously produces glucose (a process called gluconeogenesis). In Type 2 Diabetes, this process is overactive, contributing to high fasting blood sugar. Metformin's primary action is to suppress this hepatic glucose production, which is why it is particularly effective at lowering fasting blood glucose levels.

2. Improving Insulin Sensitivity

Metformin makes muscle and fat cells more responsive to insulin, allowing them to take up glucose more efficiently. This addresses the core problem in type 2 diabetes — insulin resistance.

3. Slowing Intestinal Glucose Absorption

Metformin modestly slows the absorption of glucose from the intestine after meals, contributing to lower post-meal blood sugar spikes.

What Metformin Does Not Do

Understanding metformin's limitations is as important as knowing its benefits:

  • It does not cause hypoglycemia when used alone — because it does not stimulate insulin release, blood sugar cannot drop dangerously low from metformin alone
  • It does not cause weight gain — unlike many older diabetes medications, metformin is weight-neutral or may cause modest weight loss
  • It does not treat type 1 diabetes — metformin requires some residual insulin production to work

Dosing

Metformin is available in immediate-release (IR) and extended-release (ER/XR) formulations.

Side Effects

Common (Gastrointestinal)

The most common side effects are gastrointestinal and affect up to 30% of patients, especially when starting treatment:

  • Nausea
  • Diarrhea
  • Stomach upset or cramping
  • Metallic taste in the mouth

These effects are usually temporary and improve within 2–4 weeks. Starting at a low dose and gradually increasing (as prescribed) minimizes them significantly. Switching from immediate-release to extended-release formulation also reduces GI side effects for many patients.

Vitamin B12 Deficiency

Long-term metformin use reduces the absorption of vitamin B12 from the gut. This can lead to B12 deficiency over time, which may cause:

  • Fatigue
  • Numbness or tingling in hands and feet
  • Cognitive changes

Recommendation: Ask your doctor to check your B12 level annually if you have been on metformin for more than 2 years.

Lactic Acidosis (Rare but Serious)

Lactic acidosis — a dangerous buildup of lactic acid in the blood — is a rare but serious risk associated with metformin. It is most likely to occur in patients with:

  • Significant kidney impairment (metformin is contraindicated when eGFR < 30 mL/min/1.73m²)
  • Severe liver disease
  • Heart failure
  • Situations of acute illness, dehydration, or contrast dye procedures

Symptoms include muscle pain, weakness, difficulty breathing, and abdominal discomfort. This is a medical emergency.

Drug Interactions

Metformin has relatively few drug interactions compared to other diabetes medications. The most important:

  • Contrast dye (iodinated): Metformin should be held before and after procedures using IV contrast dye due to the risk of acute kidney injury and subsequent lactic acidosis. Your doctor will provide specific instructions.
  • Alcohol: Heavy alcohol use increases the risk of lactic acidosis and should be avoided.
  • Certain medications that affect kidney function (NSAIDs, ACE inhibitors in high doses) may require dose adjustment.

Who Should Not Take Metformin

Metformin is contraindicated in:

  • Severe kidney disease (eGFR < 30 mL/min/1.73m²)
  • Active liver disease or cirrhosis
  • History of lactic acidosis
  • Diabetic ketoacidosis

Metformin Beyond Diabetes

Metformin is being studied for a range of conditions beyond type 2 diabetes:

  • Polycystic ovary syndrome (PCOS): Widely used off-label to improve insulin sensitivity and restore ovulation
  • Prediabetes prevention: The Diabetes Prevention Program showed metformin reduced progression to type 2 diabetes by 31%
  • Anti-aging research: Ongoing trials are investigating metformin's potential to slow aging-related processes
  • Cancer prevention: Observational data suggests metformin users have lower rates of certain cancers, though causality is unproven
