Overview
Metformin and Ozempic (semaglutide) are two of the most prescribed medications for type 2 diabetes in the United States — but they are fundamentally different drugs that work through entirely different mechanisms, come at vastly different price points, and are appropriate for different patient profiles.
This article compares them across every clinically relevant dimension: mechanism, efficacy, weight effects, cardiovascular outcomes, side effects, cost, and when each is the right choice.

How They Work
Metformin
Metformin works primarily by suppressing hepatic glucose production — the liver's tendency to release stored glucose into the bloodstream even when blood sugar is already elevated. It also improves insulin sensitivity in muscle and fat cells, and modestly slows intestinal glucose absorption.
Crucially, metformin does not stimulate insulin release, which is why it carries almost no risk of hypoglycemia when used alone.
Ozempic (Semaglutide)
Ozempic is a GLP-1 receptor agonist — a synthetic version of the hormone glucagon-like peptide-1, which is naturally released from the gut after eating. It works through three mechanisms simultaneously:
- Stimulates glucose-dependent insulin release — the pancreas releases more insulin, but only when blood sugar is elevated, preventing hypoglycemia
- Suppresses glucagon — reduces the liver's glucose output
- Slows gastric emptying and reduces appetite — food leaves the stomach more slowly, and appetite signals in the brain are dampened
This third mechanism is why Ozempic causes significant weight loss — an effect metformin does not reliably produce.
Blood Sugar Control
Both medications reduce HbA1c (the 3-month average blood sugar marker), but Ozempic is generally more potent:
- Metformin: Reduces HbA1c by approximately 1.0–1.5 percentage points
- Ozempic 0.5mg: Reduces HbA1c by approximately 1.4 percentage points
- Ozempic 1mg: Reduces HbA1c by approximately 1.6 percentage points
For patients who start with very high HbA1c (above 9%), Ozempic's greater potency may be clinically meaningful. For patients near target (HbA1c 7.0–8.0%), metformin's efficacy is often sufficient.
Weight Loss
This is where the two drugs diverge most significantly:
Metformin is weight-neutral to modestly weight-reducing. Studies show an average weight loss of 2–3 kg over 12–24 months — meaningful but not transformative.
Ozempic produces clinically significant weight loss. In the SUSTAIN trials, patients on semaglutide 1mg lost an average of 4–6 kg over 30 weeks. At the higher 2.4mg dose (approved as Wegovy for obesity), average weight loss exceeds 15% of body weight.
For patients with Type 2 Diabetes and Obesity — a very common combination — Ozempic's weight loss benefit is a major advantage that metformin cannot match.
Cardiovascular Outcomes
This is one of the most important clinical distinctions between the two drugs.
Ozempic has demonstrated proven cardiovascular benefit in the SUSTAIN-6 trial (a large, randomized controlled trial): patients with established cardiovascular disease who took Semaglutide had a 26% relative reduction in the risk of major adverse cardiovascular events (heart attack, stroke, cardiovascular death) compared to placebo.
Metformin has observational data suggesting cardiovascular benefit, and the UKPDS trial showed reduced cardiovascular events in overweight patients with newly diagnosed diabetes. However, metformin has not been tested in a dedicated cardiovascular outcomes trial the way GLP-1 drugs have.
For patients with established heart disease or high cardiovascular risk, clinical guidelines (ADA, ESC) now recommend adding a GLP-1 agonist like semaglutide regardless of HbA1c level.
Side Effects
Metformin
- GI effects (nausea, diarrhea, stomach upset) are common, especially when starting — affect ~30% of patients
- Usually improve within 2–4 weeks; extended-release formulation significantly reduces GI side effects
- Vitamin B12 deficiency with long-term use — annual monitoring recommended
- Lactic acidosis is rare but serious; contraindicated in severe kidney disease (eGFR < 30)
Ozempic
- Nausea and vomiting are common, especially during dose escalation — affect 30–50% of patients
- Diarrhea or constipation in ~10–20% of patients
- Injection site reactions (mild, usually resolve)
- Pancreatitis — rare but reported; avoid in patients with history of pancreatitis
- Contraindicated in patients with personal or family history of medullary thyroid carcinoma or MEN2
Cost
This is the starkest difference between the two medications:
Metformin is one of the cheapest medications in existence. Generic metformin costs $4–$15 per month at most pharmacies, and is available on the $4 generic lists at Walmart, Costco, and many other chains.
Ozempic costs approximately $850–$1,000 per month without insurance. With insurance coverage for diabetes (not weight loss), most patients pay $25–$150/month depending on their plan. Novo Nordisk's savings program can reduce costs to $99/month for eligible commercially insured patients.
For uninsured or underinsured patients, cost is a decisive factor in favor of metformin.
When Is Each Drug the Right Choice?
Choose Metformin when:
- Newly diagnosed type 2 diabetes with HbA1c near target (7.0–8.5%)
- Cost is a significant concern
- No established cardiovascular disease or high CV risk
- Patient prefers oral medication over injections
- Kidney function is adequate (eGFR ≥ 30)
Choose Ozempic (or another GLP-1) when:
- Established cardiovascular disease (heart attack, stroke, peripheral artery disease)
- High cardiovascular risk (multiple risk factors)
- Significant obesity alongside diabetes (BMI ≥ 30)
- HbA1c remains above target on metformin alone
- Patient is motivated by weight loss as a secondary goal
Use Both Together:
Metformin and Ozempic are frequently prescribed together — they have complementary mechanisms and additive effects on blood sugar. Current ADA guidelines recommend metformin as the foundational first-line agent, with GLP-1 agonists added when additional glucose control, weight loss, or cardiovascular protection is needed.
The Bottom Line
Metformin and Ozempic are not competitors — they are complementary tools for different clinical situations. Metformin is the right starting point for most newly diagnosed patients: it is effective, safe, inexpensive, and has decades of real-world data. Ozempic becomes the right choice — or a powerful addition — when cardiovascular risk, obesity, or inadequate glucose control on metformin alone enters the picture.
The decision should always be made with your prescriber, taking into account your full medical history, kidney function, insurance coverage, and personal preferences.
