Why You Cannot Stop Beta-Blockers Suddenly
Beta-blockers work by blocking the effects of adrenaline on the heart and blood vessels. Over time, the body compensates for this blockade by upregulating (increasing) the number and sensitivity of beta-adrenergic receptors. When a beta-blocker is stopped abruptly, these now-hypersensitive receptors are suddenly exposed to normal levels of adrenaline — causing an exaggerated response.
This "rebound" effect can cause:
- Rapid heart rate (tachycardia) — sometimes severe
- Severe hypertension (rebound [high blood pressure](/conditions/hypertension))
- Worsening angina (chest pain)
- Myocardial infarction (heart attack) — particularly in patients with coronary artery disease
- Arrhythmias
The risk of serious rebound effects is highest in patients with coronary artery disease, [heart failure](/conditions/heart-failure), or hypertension who have been on [beta-blockers](/drug-classes/beta-blockers) for months or years. Even patients on beta-blockers for migraine prevention or [anxiety](/conditions/anxiety) should taper rather than stop abruptly.
The Safe Tapering Protocol
There is no single universally agreed-upon tapering schedule, but the general principle is to reduce the dose gradually over at least 1–2 weeks, with longer tapers for higher doses or longer duration of use.

Example Taper for Metoprolol Succinate 100 mg
- Weeks 1–2: Reduce to 50 mg once daily
- Weeks 3–4: Reduce to 25 mg once daily
- Weeks 5–6: Reduce to 25 mg every other day
- Week 7: Discontinue
When Is It Safe to Stop Beta-Blockers?
There are situations where stopping a beta-blocker is appropriate — but the decision should always be made with your prescriber:
- Side effects (fatigue, sexual dysfunction, cold extremities) that significantly impair quality of life
- Switching to a different antihypertensive medication
- Blood pressure now well-controlled with lifestyle changes
- Beta-blocker prescribed for a condition that has resolved
There are also situations where beta-blockers should generally not be stopped:
- After a heart attack — beta-blockers should be continued for at least 1–3 years post-MI
- Heart failure with reduced ejection fraction — beta-blockers are life-saving; stopping significantly increases mortality risk
- Atrial fibrillation — stopping may cause rapid ventricular rate
What to Monitor During the Taper
During a beta-blocker taper, monitor for:
- Heart rate — check pulse daily; report rates above 100 bpm at rest
- Blood pressure — check twice daily if hypertensive
- Chest pain or pressure — seek immediate medical attention
- Palpitations or irregular heartbeat
- Shortness of breath
Emergency: What to Do If You Accidentally Stopped Abruptly
If you have stopped a beta-blocker suddenly and are experiencing chest pain, severe palpitations, or very high blood pressure, seek emergency medical care immediately. If you are asymptomatic, contact your prescriber as soon as possible to restart the medication and begin a proper taper.
