Metformin Dosage Guide: Starting Dose, Maximum Dose, and How to Titrate

Metformin dosing is not one-size-fits-all. Starting at the right dose and titrating slowly is the key to minimizing GI side effects. Here is a complete dosage reference for type 2 diabetes and other uses.

Why Metformin Dosing Matters

Metformin is highly effective for [type 2 diabetes](/conditions/type-2-diabetes), but GI side effects (nausea, diarrhea, stomach upset) are the most common reason patients stop taking it. The key to minimizing these side effects is starting at a low dose and titrating slowly — not jumping straight to the maximum dose. Taking [metformin](/drugs/metformin) with food also significantly reduces GI side effects.

Standard Dosing for Type 2 Diabetes

Extended-Release vs Immediate-Release

Extended-release metformin (Glucophage XR, Fortamet, Glumetza) has the same efficacy as immediate-release but causes significantly fewer GI side effects. It is taken once daily with the evening meal. For patients who cannot tolerate immediate-release metformin due to GI side effects, switching to the extended-release formulation often resolves the problem.

Renal Dosing: When to Reduce or Stop

Metformin is renally eliminated and can accumulate in patients with kidney disease, increasing the risk of lactic acidosis. Current FDA guidance:

Metformin for Prediabetes (Off-Label)

The Diabetes Prevention Program (DPP) trial showed that metformin 850 mg twice daily reduced the risk of progression from prediabetes to type 2 diabetes by 31% (vs. 58% with intensive lifestyle intervention). The ADA recommends considering metformin for prediabetes in patients with BMI ≥35, age <60, or prior gestational diabetes.

Metformin for PCOS (Off-Label)

For polycystic ovary syndrome, typical doses are 500 mg 2–3 times daily or 1,500–2,000 mg/day of extended-release. The goal is to improve insulin sensitivity, restore menstrual regularity, and reduce androgen levels.

Important Dosing Notes

  • Always take metformin with food to minimize GI side effects
  • Hold metformin for 48 hours before and after contrast dye procedures (CT scans with IV contrast)
  • Hold metformin if you develop acute illness with dehydration, vomiting, or diarrhea
  • Check renal function (eGFR) at least annually; more frequently if CKD is present
  • Supplement with vitamin B12 if taking metformin long-term (metformin reduces B12 absorption)
