Concomitant use of diclofenac with lithium can significantly increase serum lithium levels, leading to an elevated risk of lithium toxicity. Patients may experience symptoms such as tremor, confusion, ataxia, and in severe cases, seizures or coma.
Mechanism: Diclofenac, an NSAID, can reduce renal clearance of lithium by inhibiting prostaglandin synthesis, which affects renal blood flow and tubular reabsorption of lithium. This leads to decreased lithium excretion and increased serum concentrations.
