Fluoxetine Hydrochloride and Phenelzine Sulfate must not be used together. This combination is classified as CONTRAINDICATED.
The co-administration of fluoxetine and phenelzine is absolutely contraindicated due to a high risk of developing serotonin syndrome, a potentially life-threatening condition. Symptoms can include mental status changes, autonomic instability, and neuromuscular abnormalities.
Mechanism: Fluoxetine increases serotonin levels by inhibiting its reuptake, while phenelzine, an MAOI, prevents the metabolic breakdown of serotonin. This synergistic effect leads to excessive serotonin accumulation in the central nervous system.

Phenelzine Sulfate
INDICATIONS AND USAGE Phenelzine sulfate has been found to be effective in depressed patients clinically characterized as "atypical," "nonendogenous," or "neurotic." These patients often have mixed anxiety and depression and phobic or hypochondriacal features. There is less conclusive evidence of its usefulness with severely depressed patients with endogenous features. Phenelzine sulfate should rarely be the first antidepressant drug used. Rather, it is more suitable for use with patients who have failed to respond to the drugs more commonly used for these conditions.
