Combining Ketorolac Tromethamine and Rivaroxaban is a major interaction and should generally be avoided without close medical supervision.
Combining ketorolac with rivaroxaban significantly increases your risk of bleeding, including serious internal bleeding. This combination should generally be avoided.
Mechanism: Ketorolac, a non-steroidal anti-inflammatory drug (NSAID), inhibits platelet aggregation and can cause gastrointestinal irritation and ulceration. Rivaroxaban is a direct oral anticoagulant (DOAC) that inhibits Factor Xa, preventing clot formation. The concurrent use of these agents results in additive antiplatelet and anticoagulant effects, impairing hemostasis.

Rivaroxaban
Rivaroxaban is a selective inhibitor of FXa. It does not require a cofactor (such as Anti-thrombin III) for activity. Rivaroxaban inhibits free FXa and prothrombinase activity. Rivaroxaban has no direct effect on platelet aggregation, but indirectly inhibits platelet aggregation induced by thrombin. By inhibiting FXa, rivaroxaban decreases thrombin generation.
