Mechanism: Both tapentadol, an opioid analgesic, and tizanidine, a centrally acting alpha-2 adrenergic agonist muscle relaxant, cause central nervous system (CNS) depression. The co-administration of these agents results in an additive depressant effect on the CNS, leading to enhanced sedation and respiratory depression.

Tizanidine is a central alpha-2-adrenergic receptor agonist and presumably reduces spasticity by increasing presynaptic inhibition of motor neurons. The effects of tizanidine are greatest on polysynaptic pathways. The effect of these actions is thought to reduce facilitation of spinal motor neurons.
