Patients may experience profound sedation, dizziness, confusion, and impaired psychomotor function. This additive CNS depression significantly increases the risk of respiratory depression, hypotension, and falls. In severe cases, it can lead to coma or death.
Mechanism: Both tramadol and tizanidine cause central nervous system (CNS) depression. Tramadol exerts its analgesic effect through opioid receptor agonism and inhibition of norepinephrine and serotonin reuptake, contributing to CNS depression. Tizanidine is an alpha-2 adrenergic agonist that reduces spasticity by increasing presynaptic inhibition of motor neurons in the spinal cord, also leading to significant CNS depression. The concurrent use results in an additive depressant effect on the CNS.
