Empagliflozin (Jardiance) is a medication that helps your kidneys remove extra sugar from your body through urine. Empagliflozin and canagliflozin are both SGLT2 inhibitors. Concomitant use of these agents results in therapeutic duplication, leading to an increased risk of SGLT2 inhibitor-related adverse effects without providing additional glycemic control benefits. This combination is generally not recommended due to increased risk of adverse effects.
Mechanism
Both empagliflozin and canagliflozin inhibit the sodium-glucose co-transporter 2 (SGLT2) in the renal tubules, reducing glucose reabsorption and increasing urinary glucose excretion. Using both agents simultaneously leads to an additive pharmacological effect.
Clinical Management
Concomitant use of empagliflozin and canagliflozin should be avoided. If a patient is not achieving adequate glycemic control on one SGLT2 inhibitor, consider optimizing the dose of the current SGLT2 inhibitor or adding an agent from a different drug class. Patients should be monitored for signs and symptoms of dehydration, hypotension, genitourinary infections, and diabetic ketoacidosis if this combination is inadvertently used.
